Home Care is often necessary to assist and enhance a person's desire to remain safely at home following hospitalization or when living with a chronic condition.

LifeCare Medical Center's Home Care services can provide that little bit of care necessary to assist a person in maintaining or improving the ability to have an independent lifestyle.

"At LifeCare Home Care, we know the importance of receiving care at home," says Julie Pahlen, RN, PHN, Director of Home Care Services. "It's our goal to assist clients in their own homes to maximize their independence and improve their quality of life each and every day."

Services and assistance can come from nurses, home health aides, and social workers as well as physical, occupational, and speech therapists.

Home Care services range from evaluation and treatment to support, counseling, and even assistance with daily medications. Services are available to people of all ages who are recovering from illness, injury, or surgery as well as the disabled and those living with chronic illness.

"It's important for people to know they can request these services on their own behalf," Pahlen says. "Often times, referrals come from physicians, but also may be requested by a family member, friend, or directly from the patient."
